There are a lot of ways to find good theater forms. You might want to visit

1. MIT's page it is about 83 pages and has all the forms that you will ever need.

Hi. Many years back, I received a brother's collections of very useful documentation/forms for production (Ever thankful). These helped in the organizing of the play and contain materials such as Prop loan/acquire sheets, cast interview papers, wardrobe and so on... Filing these were simple and organized. It has been years since and I am to put together a 40 minutes evangelistic play in time for Christmas. I’m a lay man at this. I seem to have misplaced these helpful materials from years back. I would appreciate all advices and assistance on this. For His Glory. Jo
